What is a normal chest

The chest x-ray above shows a healthy and free lung which is darker in color due to the air in the lung compared to the white centre depicting the blood or liquid filled heart. So the presence of abnormal fluid in the chest can be seen as whiter areas.

what is pleural effusion

It is an abnormal collection of fluid in the space between the lung and chest wall. This reduces the space for free movement of lung resulting in compression of the lung. The chest x-ray appear as white lung due to the fluid and may be partly white or completely white.

symptoms of pleural effusion

The fluid in the chest cavity compresses the lung, reducing its function of absorbing oxygen from the air we breathe depending on the amount of fluid present. This leads to breathlessness depending on the amount of fluid collected and occasionally chest with pain with heaviness. The Chest X-ray shows the fluid as white shadow which may be complete or partial.

Natural history of pleural effusion

Depending on the cause which can be due to infection(bacteria), tuberculosis, heart failure, canceretc the cause needs to be treated.it is advisable to consult your physician who will advise the necessary investigations, most of the noncancerous cases resolve with the appropriate medical therapy. In some individual the fluid gets infected resulting in empyema. If the empyema is neglected it can result in a thick layer developing over the compressed lung resulting to trapping of the lung.

Minor procedure for treatment

The fluid can be drained with a needle or by placing a small-bore catheterfor relief of breathlessness as well as analysis of the fluid for microbiology, biochemistry, cytology and cell block. For infected fluid causing empyema if drained early it can prevent the development of trapped lung. Once the fluid organizes and cause a trapped lung it requires a surgical intervention called Decortication.

Trapped lung surgery (Decortication) traditionally required a big incision on the chest known as thoracotomy. Nowadays with modern video visualization system it can be done through one or two small incision known as VATS or thoracoscopy.
